Benefits

 Online Benefits Benefit eligible employees can enroll in benefits online. Your ID is the first 6 letters of your last name followed by the first letter of your first name, followed by your month and date of birth (i.e., Jane Dovebar, DOB 2/10/69 would be dovebaj0210). The default password is your social security number (which you will be asked to change after you log in). If you have problems logging in call 877.396.3800 x222-5936.

Health

 

Medical Insurance

All full- and part-time employees (who work a regular schedule of sixteen (16) hours or more per week) and their dependents are eligible for medical insurance coverage on the first of the month following 1 full month of employment.

Dental Insurance

All full- and part-time employees (who work a regular schedule of sixteen (16) hours or more per week) and their dependents are eligible for dental insurance coverage on the first of the month following 1 full month of employment.

Prescription Plan

3-tier plan providing extensive list of covered drugs with a co-pay

Vision

Corporate discount on prescription glasses, lenses and frames

Life Insurance *

1 1/2 times your annual salary paid to your beneficiary in the event of your death

Supplementary Life Insurance *

Additional life coverage, through payroll deduction

Temporary Disability

NJ State disability insurance after temporary disability,through payroll deduction

Supplementary Short Term Disability *

Additional Short Term Disability coverage may be purchased

Supplementary Long Term Disability *

Continuation of disability insurance after temporary disability, through payroll deduction

Leave of Absence *

Time off, paid or unpaid, for medical, maternity, adoption, foster care, paternity or serious health condition of self or a family member, each according to FMLA and NJFLA guidelines.

Financial Security

 
Pension A defined contribution plan for eligible employees with one year of service and have reached the age of 21. You become fully vested in our Pension Plan after five years.
Tax-Shelter Annuity Plan - 403(B) Individual tax-sheltered savings plan for your retirement; through payroll deduction
   
Tuition Reimbursement 100% reimbursement for full-time employees up to a maximum of $5,000 for graduate programs and $3,500 for undergraduate programs. Part time employees reimbursements and maximums prorated. Reimbursement will be made for courses of study directly related to the likelihood of career advancement within PHCS.
